Responsibilities
Assist with overall development, monitoring, and evaluation of the Case Management program; Provide leadership, training, mentorship, and supervision to case management leadership and administrative support staff; Support quality assurance and regulatory compliance; Evaluate case management services including referrals, intake, eligibility determination, program planning, monitoring, and assessment; Ensure required documentation, assessments, plans, reports, and financial information are completed accurately and on time; Partner with county representatives, service providers, families, guardians, and other members of individuals' support teams; Monitor service delivery and documentation to ensure contract and waiver requirements are met; Assist with hiring, staff evaluations, training, and ongoing professional development; Participate in financial planning and analysis for the department; Respond to critical and emergent situations and assist with crisis response and follow-up planning.
