Responsibilities
Complete comprehensive evaluations of members and recommend case resolution approaches; Identify high-risk factors and refer to clinical case management or crisis intervention; Provide coaching and support to empower members for independent medical and lifestyle choices; Coordinate and monitor assigned care plan activities; Consult with clinical care managers and providers to overcome barriers to member goals; Present cases at multidisciplinary rounds; Facilitate clinical hand-offs during transitions of care; Educate members about available resources and services; Identify and escalate quality of care issues; Link members to providers and social supports; Travel 10-20% locally for face-to-face member visits; Work two evenings per week until 9:00 PM
