Founding Engineer – iClinic AI - Voice AI Healthcare Startup

## About the role iClinic is a Houston-based AI healthcare startup founded by a practicing interventional cardiologist. We build purpose-built, production-grade clinical voice agents that reduce administrative burden and improve operational efficiency for clinics, doctors, and patients. We are not building generic chatbots — and we're already live in medical clinics. The code you write runs in real clinical environments — if a bug slips through, a real clinic feels it. The bar for quality, reliability, and attention to detail is high. You'll work directly with a founder-physician, own what you build end to end, and help shape the product and architecture from the ground up. We need someone with 2–3+ years of experience who is hardworking, meticulous, and thrives under real responsibility. If that sounds like pressure rather than opportunity, this isn't the right fit. ## Responsibilities - Build and improve real-time AI voice agents in live clinical settings - Design conversation logic and integrate voice pipelines end to end - Build and maintain APIs and webhook-driven workflows - Make early technical and product decisions alongside the founder ## Requirements ### Must-have - At least 2–3+ years of professional software development experience - Proven ability to build new products from scratch — detail-oriented, hardworking, full ownership - Hands-on experience with Retell AI, Twilio, and LiveKit - Strong API design and webhook integration skills ### Nice-to-have - Experience building Voice AI systems - Interest or experience in healthcare or healthtech - LLM integration, backend (Node.js/Python), AWS, or frontend (React/Next.js) - Prior startup experience ## Work Expectations Must overlap with US working hours. For India-based candidates: ~4 hours mornings IST (founder syncs), ~4 hours evenings IST (clinic hours), plus 1–2 hours on weekends.
