Perform outbound calls to patients to understand clinical needs and connect them with resources; Perform outbound calls to providers to schedule appointments or follow up on care; Answer inbound calls from patients, providers, and Village resources; Follow up with patients to ensure needs are met and schedule future check-ins; Coordinate with clinical resources and providers to ensure smooth continuum of care; Monitor patient hospitalizations and follow up with VillageHealth Nurses as necessary; Notify patients of location and appointment times; Provide patients with education materials and send letters to primary care physicians and specialists; Assist with the referral process.
