Responsibilities
Serve as onsite Evergreen representative at practice sites to explain pharmacy services and answer patient questions. Engage patients using standardized messaging to build trust and drive informed enrollment decisions. Obtain and document patient consent and capture necessary enrollment information. Update patient records in the EHR, including preferred pharmacy changes, triggering electronic prescriptions, and documenting consent or decline. Conduct telephonic outreach to patients without office visits to introduce the service and complete enrollment. Accurately document all patient interactions and coordination activities in clinical systems. Act as a liaison between patients, providers, and the pharmacy partner to ensure care continuity.
