Responsibilities
Serve as a community-based, patient-facing resource building relationships through direct in-person re-engagement; Provide hands-on support including program re-enrollment, onboarding, and mobile app setup; Conduct telephonic outreach to caregivers to re-engage children in care; Educate patients and caregivers on services and community resources; Maintain and utilize knowledge of local community resources to connect families to appropriate services; Conduct community events and facility rounding to strengthen relationships; Partner with behavioral health teams and providers for discharge planning; Collaborate with the Patient Engagement Team to support re-engagement goals; Support non-clinical care coordination efforts including connecting patients to primary care providers.
