Responsibilities
Conduct comprehensive assessments of individuals with drug and alcohol-related needs; Develop, implement and review individualised care plans; Provide medication management including supporting detoxification and withdrawal management; Provide education and support around harm reduction, relapse prevention and recovery; Support the delivery of the OOAT Pharmacotherapy Program; Identify and respond to clinical risk and changes in presentation; Liaise with external services to support continuity of care and appropriate referrals; Contribute to clinical audits, quality improvement and service development activities
