Responsibilities
Conduct and coordinate clinical services including ECGs, Spirometry, ABI testing and DMMRs; Coordinate care with GPs, specialists, allied health providers and community services; Proactively identify eligible patients for chronic disease management and preventative health services; Maintain accurate patient records and ensure Medicare compliance requirements are met
