Responsibilities
Provide support in accordance with client Care Plans and support plans, assist clients to live safely and meaningfully in home and community environments, observe and report changes relevant to accident-related injuries or functional capacity, ensure accurate reporting and documentation for TAC requirements, escalate matters related to TAC clients to Program Leads or Coordinators
