Responsibilities
Undertake intake and assessment to build a shared understanding of participant needs and strengths; Develop individual case plans in collaboration with participants; Support participants to meet bail or parole conditions and work towards long term goals; Assist participants to access services, attend appointments and build community supports; Advocate on behalf of participants with government and non-government agencies; Maintain accurate, up to date client records and data in line with TSA standards; Monitor progress and review case plans to ensure they continue to meet participant needs; Identify risks and escalate concerns to the Team Leader as required; Contribute to a safe, structured and supportive environment within the residential facility
