Responsibilities
Perform comprehensive assessments of patients with acute and chronic wounds, ostomies, fistulas, drains, and skin-integrity concerns; Develop and implement individualized treatment plans in collaboration with physicians and the interdisciplinary team; Provide direct patient care, including advanced wound treatments and ostomy education; Conduct patient and family teaching related to wound and ostomy management, appliances, and long-term care strategies; Serve as a resource and consultant to nursing staff and clinical teams regarding wound/ostomy care best practices; Participate in product evaluation, quality improvement initiatives, and development of clinical protocols; Document clinical assessments, treatment plans, and patient outcomes in the electronic medical record; Support hospital-wide initiatives to reduce pressure injuries and improve skin-integrity outcomes
