Conduct home visits to identify pregnant women and children under five; Screen children aged 6-59 months for acute malnutrition using MUAC; Refer malnourished children and at-risk pregnant women to health facilities; Train community leaders on health and nutrition best practices; Lead community awareness sessions on breastfeeding and hygiene; Participate in vaccination and public health campaigns; Report priority diseases and epidemics to supervisors; Collect and submit health and nutrition data; Coordinate with local health structures and partners.
