Maintain constant visual and verbal contact with patients; accompany patients to bathrooms and off-unit locations; monitor for changes in behavior or suicidal thoughts; report significant changes to assigned RN; manage emergency situations; perform safety checks on restrained extremities every 15 minutes; identify and report hazardous items in the environment.
