## About Cohera Health Cohera Health is building the AI infrastructure for the future of outpatient care. Our platform closes the gap between clinical visits: an AI call agent that handles patient intake and scheduling, a medical scribe that documents every visit in real time, and a companion app that delivers the clinician's own guidance to patients in the moments that matter most between appointments. We work with behavioral health and weight loss clinics and we are growing fast. You will be the kind of engineer who ships, who cares about the patient on the other end of the product, and who wants to build something that genuinely changes how healthcare works. ## The Role We are looking for a Senior AI Engineer who has built production AI systems — not prototypes — in clinical or healthcare environments. You will own the technical architecture across all three of our products, working directly with our CTO and clinical advisors to design systems that are accurate, safe, and deeply integrated with the workflows clinicians already use. This is a high-ownership role. You will make real architectural decisions, work directly with EHR platforms, and shape how AI interacts with patients through voice and mobile. If you want to be a number on an engineering org chart, this is not the right role. If you want to build something that matters from the ground up, keep reading. ## What You Will Do - Design and maintain the AI pipeline across voice call agent, medical scribe, and companion app - Architect and own EHR integrations using standards including HL7 FHIR, with platforms such as Epic, Athenahealth, and Elation - Build and optimize voice AI systems — real-time transcription, natural language understanding, and voice response — with low latency and high accuracy in clinical audio environments - Work with LLMs (including prompt engineering, fine-tuning, RAG, and context injection) to create AI behavior that is specific to individual patients and treatment plans, not generic - Collaborate with our clinical advisor (physician) to ensure AI outputs meet clinical accuracy standards and do not cause harm - Design patient-facing UX flows in the companion app and call agent — you understand that healthcare UX has higher stakes than consumer apps and you design accordingly - Own HIPAA-compliant data architecture: PHI handling, BAA compliance, audit logging, and encryption at rest and in transit - Build evaluations and monitoring pipelines to catch model drift, hallucinations, and accuracy regressions before they reach patients ## What We Are Looking For - Direct experience in healthcare technology — you have shipped something that touches patient data or clinical workflows - Hands-on experience with EHR integrations: FHIR APIs, HL7 messaging, or integration engines such as Redox, Health Gorilla, or Metriport - Experience building voice AI products — real-time speech-to-text, conversational AI, or telephony integrations (Twilio, Vonage, or similar) - Strong LLM engineering skills: prompt engineering, RAG architecture, context window management, and evaluation frameworks - HIPAA literacy — you understand what PHI is, how to handle it, and what a BAA means - Proficiency in Python and at least one cloud platform (AWS preferred) ## What We Offer - Competitive salary - Direct access to clinical advisors and real patient workflows from day one - The chance to shape a platform being used by real clinicians and real patients right now - A small, senior team — no bureaucracy, no committees, just building ## How to Apply E-mail [email protected] a copy of your resume, or apply directly through the platform.
