Lead Full-Stack AI Engineer (iOS + Backend)

## About the role We're building a next-generation healthcare app focused on out-of-hospital care, proactive guidance, and low-friction health management. The goal is to reduce how much the patient has to actively manage themselves by creating a system that can intelligently handle reminders, care-plan follow-up, medication workflows, appointment coordination, and personalized next-step guidance in the background. We are looking for a highly capable technical lead-level engineer who can build the product end-to-end across iOS, backend systems, and AI orchestration. This is not a simple chatbot role. We want someone who can design and implement the core orchestration engine that maintains patient state, processes health data, triggers the right workflows at the right time, and creates a product that feels proactive, reliable, and genuinely useful. You will be responsible for building most of the technical foundation of the product, including the Swift-based iOS app, backend architecture, APIs, databases, health-data workflows, notification systems, and the AI orchestration layer. You should be comfortable designing systems from scratch, making product-minded technical decisions, and shipping quickly without losing structure or quality. ## What you'll work on - Build and improve the iOS app in Swift / SwiftUI - Design and implement the backend architecture and APIs - Create the orchestration engine that powers reminders, care workflows, follow-ups, and next-step actions - Build document, symptom, medication, appointment, and patient-state pipelines - Integrate AI thoughtfully into the product, especially for summarization, workflow assistance, and intelligent task coordination - Build reliable systems for notifications, scheduling logic, and patient-specific action triggers - Help shape product direction, MVP scope, and technical decisions from the ground up ## What we're looking for - Strong experience building production apps and backend systems - Comfortable owning both frontend and backend development - Strong Swift / SwiftUI experience - Strong backend experience with APIs, databases, auth, and cloud infrastructure - Experience building AI-powered products beyond simple chat interfaces - Ability to think in systems, workflows, and orchestration rather than just screens - Strong product judgment and ability to work independently ## Bonus if you have - Experience in healthtech, medtech, or regulated products - Experience with OCR, document parsing, or event-driven workflows - Experience building agentic systems or stateful orchestration engines - Familiarity with healthcare data models, scheduling flows, or patient engagement products
