Undertake child-focused casework including assessment, case planning, implementation and review; Support development and sustainability of Kinship and Foster Care placements; Provide ongoing support and supervision to carers through home visits and telephone support; Mentor carers on trauma, attachment, and complex behaviors using the CARE model; Facilitate placement transitions and support introductions; Complete carer assessments (provisional, renewal, review); Develop and monitor risk assessments and safety plans; Maintain accurate records in CIRTS database.
